About UsThe Meeting Milestones Initiative (MMI) is a community project launched to make sure all children in Grand and Jackson Counties have the opportunity to reach their highest potential. By creating a community process involving all healthcare providers and early childhood programs in the both counties, every family with young children can easily connect with local professionals to receive free screenings, seamless referrals, and cross-community follow up communication across Grand and Jackson Counties agencies.

MMI Goals:
1) Sustain our system where all Grand and Jackson Counties primary care practices and early childhood programs administer standardized screenings universally to children ages 0 to 5 in accordance with the Grand and Jackson County Community Road Map.
2) Ensure that children with a potential developmental, hearing, vision, or oral health concern are identified, receive appropriate referrals, and are evaluated and connected with services in a timely manner.
3) Make sure that screening and referral information is consistently shared between all screening and referral agencies that interact with a given child.
4) Promote the uptake of medical homes for Grand and Jackson County families.
5) Provide families with consistent, accurate messaging and appropriate action steps surrounding screening, preventive health, and child development.
